[edit] Synopsis
It's night time at Sacred Heart and everyone is bored, waiting for something interesting to happen. To pass the time, Janitor is using a new gadget he has bought/made; a small remote that can make different sounds at the press of a button - including a flat-line sound to fool doctors into thinking that a patient is crashing when they are really not, and the beeping sound of a truck reversing which he can play when someone insecure about their weight walks backwards (particularly Dr. Kelso who takes it rather personally). Dr. Cox on the other hand is playing a new game with the interns called "Answer The Question Correctly Or Stay Here Forever", which as the name suggests - the intern is asked an almost impossible question, which if answered incorrectly results in them having to "stay here forever", although even if you give the correct answer you probably still have to play.
However, things get interesting when a suicide patient comes in and is suffering from amnesia, causing him to not be able to remember parts of his past - such as his name, and also why he is in the hospital. This causes a debate over whether they should tell him that he tried to commit suicide, or allow him to forget the painful things in his past. This causes J.D. to think about a sequence of all the painful things that have happened to J.D. and others, such as tripping over and walking into walls. After Elliot tells everyone about another painful and slightly odd story about her childhood with her family, J.D. thinks about all the odd moments that have happened to him. The group then start talking about how there are experimental drugs which can erase memories and dreams, and while Turk talks about how he can only remember dreams about cheese or Tyra Banks, J.D. thinks about the fantasies he has had.
Janitor then tries to entertain everyone, but mainly himself, by fooling them with the flat-line sound again. However, everyone knows it is just his gadget making the sound because they have already seen that joke so many times. So instead he decides to dance for entertainment, and that gets J.D. having a sequence of memories of people dancing. Meanwhile, Dr. Cox is still going round torturing the interns with his new game, and this again gets J.D. thinking, this time about all the times people have been mean to each other. Dr. Cox then asks J.D. to tell his patient that he committed suicide, but when he decides he doesn't want to, Elliot tells him instead - which makes J.D. realize that it is both the good and the bad memories that make what everyone is like, as he thinks about the dramatic moments in his life and his friend's life, both the happy and the sad.

Janitor makes a device that creates the noise of a flat line or the sound of a truck reversing. He uses this to torture Dr. Kelso about his weight, by laying the truck reversing sound everytime Kelso moves backwards, as well as trying to instill some action to the occasion by shocking doctors by making fake flat-line noises. When his flat-line imitator fails to grab the attention of J.D., Turk, Elliot and Carla he then begins dancing.

- This episode was voted the worst Scrubs episode ever by www.tv.com, a 6.8
- This episode was originally supposed to be shown at some point after the episode My Words of Wisdom.
- In this episode, Dr. Cox has a shaved scalp. In the following episode "My Fishbowl" he has curly hair again, as the episodes were shown out of order. Producer Randall Winston states in the DVD commentary, that the bald head would be explained in a future episode.
- According to Producer Randall Winston in the DVD commentary, it normally takes five days to shoot a Scrubs episode. This one took three.
- According to Randall Winston in the DVD commentary, the flashback fall in which Elliot is hit by a cart, falls back and is covered in cereal took about eight hours to shoot.
- J.D. says that to Daydream you have to tilt your head to the left, and yet he tilts it to his right, which is our left.
