[edit] Synopsis

J.D.'s demented patient, Mr. Bober, can only say "pickles". J.D. mocks Mr. Bober and Dr. Cox says that the nursing home sent him to bring down his temperature and then bounce him back. His temperature is now 99.5. J.D., who is still angry about Elliot being in love with Sean calls his brother, Dan. Dan comes down because his mother is getting remarried.

Since Elliot's boyfriend is in New Zealand, Carla asks if Elliot has tried to have phone sex with him. Elliot says that she hasn't tried yet because the walls are too thin. Elliot then tries to have phone sex in the woods, but then is disrupted by Boy Scouts. Elliot asks Carla how to make money quick to go see Sean. After Dr. Kelso suspends someone for moonlighting, Elliot asks other doctors where they moonlight. Then J.D. brings Dan to work, and he meets Dr. Cox. Cox asks if Dan could possibly tail him for the day. J.D., wants him to say no, and Dr Cox does, but after Dr Cox finds out that J.D. wanted him to say no, then he changes his mind.

To make money, Carla takes Elliot to her moonlight at the animal hospital. Then Mr. Bober is back, and Cox asks why this "gomer" is back. J.D. explains to Dan that "gomer" is an old person who doesn't have the common decency to die. Dr. Cox explains that this is the attitude around the hospital.

While Carla and Elliot were moonlighting at the animal hospital, Dr. Kelso brings his dog over and catches them. Dan stays around longer because he is angry about J.D.'s attitude. They start fighting, and Dan says that they are brothers and that all that counts, but J.D. thinks otherwise.

Elliot and Carla plead to Ted not to be suspended, and Turk tells J.D. that he needs to apologize before Dan leaves. J.D. apologizes to Dan, while Elliot admits that she needs the money to visit Sean, and Carla needs the money for her wedding. Kelso changes his mind about suspending Elliot and Carla.

Dan stops to talk to Dr. Cox as he is leaving. Dan says that he is a screw-up and J.D. may never look up to him, but he knows that J.D. is going places. Dan asks Dr. Cox to take the responsibility of being J.D.'s mentor more seriously. Dr. Cox agrees with Dan and they shake hands. Carla and Elliot continue to moonlight and Mr. Bober comes back to the hospital.
