Alex has been in and out of Sacred Heart many times. Dr. Cox has been his doctor every time, but has been unable to diagnose him. After J.D. and Turk poke fun at Cox, he asks them to take Alex's urine for testing. They haphazardly leave it in the sun which turns it purple. They Google "purple pee" and diagnose Alex with Acute intermittent porphyria. When Dr. Cox tells him the news, he is glad that is manageable and that he at least knows what it is.
